Transoral Outlet Reduction, or TORe, is a minimally invasive endoscopic procedure designed specifically for patients who have previously had Roux-en-Y gastric bypass surgery and have begun to regain weight over time. After gastric bypass, the small connection between the stomach pouch and the small intestine — called the gastrojejunal outlet — can gradually stretch. When this happens, food passes through more quickly, reducing feelings of fullness and making it easier to consume larger portions.
TORe addresses this directly. Using a flexible endoscope passed through the mouth — no incisions required — a surgeon places sutures around the outlet to reduce its diameter. This tightens the connection, slows gastric emptying, and restores the sensation of fullness that helped you succeed after your original bypass surgery.

Your care team will review the details of your original gastric bypass, your current weight trajectory, and your overall health. Imaging or endoscopy may be performed to assess the degree of outlet dilation prior to treatment. TORe is performed under sedation and typically takes 30 to 60 minutes. The endoscope is guided through the mouth, past the stomach pouch, and to the gastrojejunal outlet. Sutures are then placed around the outlet opening to reduce its diameter, restoring the restriction that was present when your bypass was first performed.
Most patients are discharged the same day and able to return to normal daily activities within two to three days. You will follow a liquid and soft food progression for the first one to two weeks as the sutures heal.
