Transoral Outlet Reduction (TORe): Regain Control After Gastric Bypass

What is Transoral Outlet Reduction (TORe)?

Transoral Outlet Reduction, or TORe, is a minimally invasive endoscopic procedure designed specifically for patients who have previously had Roux-en-Y gastric bypass surgery and have begun to regain weight over time. After gastric bypass, the small connection between the stomach pouch and the small intestine — called the gastrojejunal outlet — can gradually stretch. When this happens, food passes through more quickly, reducing feelings of fullness and making it easier to consume larger portions.

TORe addresses this directly. Using a flexible endoscope passed through the mouth — no incisions required — a surgeon places sutures around the outlet to reduce its diameter. This tightens the connection, slows gastric emptying, and restores the sensation of fullness that helped you succeed after your original bypass surgery.

Recovery is typically very quick. Many patients resume their normal routine within a few days. TORe is an established, evidence-based procedure that can provide meaningful, durable support for patients committed to getting back on track with their weight-loss goals.

How the Procedure Works

Here is what to expect at each stage of the TORe process:

Before the Procedure

Your care team will review the details of your original gastric bypass, your current weight trajectory, and your overall health. Imaging or endoscopy may be performed to assess the degree of outlet dilation prior to treatment. You will also receive dietary and fasting instructions to follow in the days leading up to the procedure.

During the Procedure

TORe is performed under sedation and typically takes 30 to 60 minutes. The endoscope is guided through the mouth, past the stomach pouch, and to the gastrojejunal outlet. Sutures are then placed around the outlet opening to reduce its diameter, restoring the restriction that was present when your bypass was first performed.

After the Procedure

Most patients are discharged the same day and able to return to normal daily activities within two to three days. You will follow a liquid and soft food progression for the first one to two weeks as the sutures heal. Our team will work with you on dietary coaching and follow-up support to help maximise your results following the procedure.

Am I a Good Candidate for TORe?

TORe may be the right next step if you:

  • Have previously had Roux-en-Y gastric bypass surgery
  • Have experienced significant weight regain after your bypass
  • Have been assessed and found to have a dilated gastrojejunal outlet
  • Are committed to lifestyle and dietary changes alongside the procedure
  • Want to avoid revisional bariatric surgery
  • Are in generally good health suitable for endoscopic procedures

A thorough consultation with our team will confirm whether TORe is appropriate for your specific situation and what realistic outcomes you can expect.

Benefits of TORe

Studies show TORe patients lose an average of 8 to 10 percent of total body weight. The benefits extend beyond weight loss alone:

Restored Restriction

TORe effectively recreates the anatomical restriction from your original bypass, helping you feel full faster and eat less at each meal without repeat surgery.

Avoids Revisional Surgery

TORe offers a non-surgical path to addressing weight regain, sparing patients from the significantly higher risks and recovery demands of revisional bariatric surgery.

Quick Return to Normal Life

With no external incisions and minimal recovery time, most TORe patients are back to work and daily routines within just a few days of the procedure.

Risks & Considerations

TORe is a safe and well-tolerated procedure, but it is important that every patient has a clear understanding of the potential risks involved. We take time during your consultation to cover all of this in detail so you feel fully prepared.

Common short-term side effects include nausea, mild abdominal pain, and difficulty swallowing in the first few days as the sutures settle. Serious complications such as bleeding, suture failure, or injury to surrounding tissue are uncommon but possible, and we monitor all patients closely after the procedure.

TORe is most effective when combined with a renewed commitment to dietary habits and lifestyle changes. Sutures alone will not produce lasting results without the behavioural foundation to support them. Our team provides comprehensive follow-up support to help you build and maintain those changes.
